  • Maintain full responsibility for a caseload of  50 to 75 cases depending on program and fiscal requirements

  • Ensure that all needed core services are delivered for each clients on the caseload a minimum of 1 time per month

  • Provide services to clients as needed

  • Document the billable and non-billable care services on the progress note form, and enter other services as needed.  Be specific and include comments

  • Complete accurate and up-to-date case lists, case notes, and an activity log on a daily basis

  • Submit daily logs and progress notes to Data Entry Staff weekly

  • Complete intakes in a timely manner, track, and submit immediately to your Care Manager

  • Case conference with and obtain  necessary records  from all primary agencies that are involved with the clients

  • Provide appropriate referrals and ensure follow-up by monitoring the quality of services, verifying and ensuring client participation

  • Provide education and supportive counseling to ensure that clients understand and follow up with services to which they are referred

  • Conduct home visits and fieldwork on an ongoing basis and as needed

  • Routinely conduct outreach activities for both missing and new clients

  • Participate in the agency quality improvement and professional development programs, attending internal and external training courses and committees



  • Masters or Bachelors Degree preferred in health or human services related fields or Associate’s Degree in health or human services related fields or certification as an RN or LPN.  Bilingual Spanish / English preferred.

  • Experience or interest working with HIV/AIDS, chronic illnesses, substance abuse, mental illness, and homelessness.

  • Awareness of and sensitivity to cultural and socioeconomic characteristics of populations served.

  • Ability to work collaboratively with other professionals.

  • Ability to set appropriate limits with clients and co-workers.

  • Excellent writing and oral communication skills.

  • Good management and organizational skills. Basic computer skills required. Able to work 35 hours per week, Monday through Friday during normal business hours, or as needed to carry out the job responsibilities.
