The Case Manager provides supportive and direct intensive case management services to the homeless families residing in the Shelter. The Case Manager assesses the client’s needs and jointly develops a collaborative plan that addresses those needs. The Case Manager prepares the client for self sufficiency and independent living and supports their transition back into the community. The Case Manager engages all clients and conferences with them when issues of noncompliance are identified. The Case Manager organizes case conferences and ensures that their clients are actively involved in their discharge planning.
Bachelor’s degree in Social Work, Psychology or a related field.
Experience working with clients pursing independence and self sufficiency outcomes is preferred.
